Get into the spirit of Cornwall Central Middle School. Mark down your order on this form and make checks payable to “CCMS PTO”. Please place your order form and payment in an envelope marked “CCMS SPIRIT WEAR” and bring to the Main Office. Items will be ordered and delivered to your child in their homeroom. Circle the color you would like in the sweatshirts, t-shirts and lounge pants. Please make sure you put your phone number in case we need to get in touch with you.
